Alternatively:
- Call the office to have forms mailed or emailed to you
- Drop by the office during working hours to pick up physical copies
Please ensure that you have completed and submitted your new patient paperwork two weeks prior to your appointment.
If you have had x-rays from your prior dentist, we use these two weeks to request them and other pertinent information to better serve your needs. The x-rays are very important as it ensures the doctor has the ability to complete the exam; without prior x-rays we may need to take them at your appointment.
